Abstract


The aims of this study were: 1) to review the study method of bacteriologic caries activity test, and 2) to compare the caries predictive ability among Alban tests, AHSM test(caries activity test with acidic high-sucrose liquid medium), and AHSM-S test (caries activity test with acidic high-sucrose liquid medium containing sulfisomidine). The comparison of caries predictive ability among the three tests are accomplished by evaluating the validity, the reliability, and the correlation with past caries experience The sensitivity, specificity, predict value, hit rate, and efficiency were computed to estimate the validity and the reliability. The caries experience and caries activity of 150 primary school children were examined. The caries examination was repeated after 12 months and the caries increment was calculated for each child. Validating criteria were five or more DdFfT and eight or more DdFfT in validity analysis, and one or more new DdFfT and two or more new DdFfT in reliability analysis. In sum of subject, the most high correlation with past caries experience was showed in the test values of 72hours incubation AHSM-S. In each grade, there was significant correlation between past caries experience and AHSM-S test value. But partially; AHSM and Alban test values were not significantly .Correlated to past caries experience. The validity of prediction. varied according to validating criterion. In degree of validity, it was estimated that 1, 2 of Alban test value(24, 72 hours incubation) and 1, 2 of Alban test value(72, 96 hours incubation) were comparatively high. In reliability analysis, AHSM test value has been shown to, be unreliable. The highest reliability was estimated in test value 2 of Alban(validating criterion : 1 or more of new DdFfT) and test value 4.2 of 72 hours incubation AHSM(validating criterion : 2 or more of new DdFfT).
Therefore, the desirable screening criterion for caries prediction was test value 4.2- 4.6 of 48, 72 hours incubation in AHSM test and test value 1, 2 of 72, 96 hour¢¥s incubation in Alba test. To evaluate the caries predictive ability of a caries activity test it seems that it is necessary to calculate sensitivity, specificity, predict value, and hit rate as well as correlation between caries
